Full-time, day shift, Rotating Weekends and Holidays
Responsibilities of LPN Care Coordinator:
- Supervision and support of licensed and certified direct care staff
- Work the floor when needed.
- Act as a resource for clinical documentation and assessments
- Oversee implementation of physician’s orders and clinical interventions
- Monitor staff compliance with clinical standards of practice and company policies
Qualifications of LPN Care Coordinator:
- Valid Kansas PN license required
- Experience in adult care and services
- Strong leadership and communication skills
- Ability to collaborate with a team
- Previous supervisory experience highly preferred
